بے نظیر نشونماپروگرام رجسٹریشن کروانے کے لیے اپ تحصیل ڈسٹرکٹ ہیڈ کوارٹر ہسپتال میں قائم کردہ مرکز پر جا سکتی ہیں وہاں پر اپ کو وہ شرائط کے مطابق اپنی رجسٹریشن کا عمل مکمل کروانا ہوتا ہے اگر اپ اہلیت کے معیار پر پورا اترتی ہیں تو وہاں پر اپ کو رجسٹریشن فارم حاصل کرنا ہے اس میں اپنی اور بچوں کے بعد رجسٹریشن فارم کو واپس جمع کروا دینا ہے تو اپ کو بتایا جائے گا رجسٹرڈ نمبر کے ذریعے کہ اپ کے رجسٹریشن کا عمل کہاں پہنچائے اور اپ اہلیت کو چیک کرنے کے لیے بھی ان ڈسٹرکٹ ہیڈ کوارٹر ہسپتال میں جا سکتی ہیں
